Sorry but this is going to be a somewhat brutal list.
SPI flash CS pin is not connected to the RP2040.
VREG_OUT is only connected to a capacitor and nothing else. DVDD is the RP2040 core supply so it being unpowered means the chip won't start up.
ADC_AVDD is only connected to a capacitor and nothing else. It should be tied to IOVDD if not needed.
IOVDD and USB_VDD shouldn't be connected to DVDD. USB_VDD needs to be 3.3V and IOVDD is whatever the I/O voltage is for this board so probably 3.3V as well.
TESTEN pin should be tied to ground.
Decoupling capacitors are bunched together and far away from the RP2040. They need to be very close to the RP2040 power pins.
Ground pad only has a single thin trace. Current path is half way across the board and then back again.
