So it's looking to me, seems confirmed by your testing, and backed-up by latest datasheet wording, that the BOOTSEL button connection and circuitry shown in the Hardware Design Guides is correct and not problematic. It's the assertions that /CS must track VCC which appear to be incorrect.

The reality is different, it can be problematic.

So that would make the design presented in the Hardware Design Guides flawed to some extent and the process of "plug it in to USB while holding the BOOTSEL button" is not as sound or as reliable as it may have seemed to be. And we don't seem to know exactly what consequences doing that might have.

I think someone needs to raise this as an issue to get some definitive and authoritative answer from Raspberry Pi as to what the actual situation is.
