I have the Pimoroni NVMe Base with an active cooler, with a audio DAC HAT on top. This coupled with an always on desktop usage load, translates to no noticeable fan noise. Pi lurks behind my monitor on the corner of the desk, approximately 2 feet away from me. I have a top "plate" to protect against conductive crap getting dropped into/on the Pi.
I suspect that the top half of a heatsink case would work for most load use cases, without throttling.
