I managed to make the virtual disk truly read-only. This has the benefit of macOS now not complaining if the Pico is pulled out without unmounting it first. This will also allow me to try with further MSC SCSI commands to tell the host OS that the disk has been changed. If I get that working, then there should be a safe way to change the contents of the partitions (or other virtual files) on the fly and then telling the host OS to flush its cache...

A small complication with this version is that it requires a small fully backwards-compatible change to TinyUSB. Therefore the project uses (hopefully temporarily) a custom version of TinyUSB, implemented as a Git submodule. I've opened a PR to TinyUSB, but I don't know when it will be merged, if ever:
