Does it only work with a VOIP handset or do you plug a "normal" PSTN handset into your router (or some other conversion box)?
For the former you'd need to identify protocol and settings and replace the handset with appropriate hardware and software (which may include duplicating the functionality of a handset). Assuming virgin let you use alternative VOIP stuff with their servers that is.
For the latter, get hold of an old modem that supports caller ID, connect it in parallel to the handset and write some software to read caller ID and hang up when necessary. This assumes that the router passes caller ID info on to the connected POTS port/device.
TBH, the first thing I'd do is ask virgin if they offer a service to block everything except whitelisted incoming numbers. And get your relative listed as a vulnerable customer too. That's supposed to reduce spam calls.
