i suspect it's got something to do with the "hdmi adapter"
in some cases, HDMI adapters do not pass the EDID data from the monitor that tells the PI what resolution to set.
or cannot handle the resolution mandated by the EDID data from the monitor....

however, your screen image says "edid=OK" for port 1. so i'm confused.

BUT: you can set the desired resolution manually.

in /boot/cmdline.txt (add to the front/end of the line, NOT a new line)
-- IMPORTANT NOTE: for Bookworm, the file is at /boot/firmware/cmdline.txt !!
video=HDMI-A-1:1024x768@60D
(or whatever resolution you want)
(above means 1024x768, 60Hz)

+1 and I would add: @bpingh what make/model of display (or TV) are you using just in case it's one with known issues/cures?
Eg. I have a Samsung TV which had an odd "natural"/max res. display setting and could not use it at that setting but, in my case, that's now possible - you might just be "not so lucky".

Added that to the back of cmdline.txt, I get to the welcome screen of the raspberry pi desktop now with a bunch of text on the bottom left, but after that screen it just goes to black, sometimes I lose signal but it usually just stays black. And I am using the Samsung odyssey g5.
